Information collected
The enquiry form asks for a name, postcode, one contact method and an optional short description. Do not provide medical history, payment information or other sensitive personal information. Technical security logs may be generated by Cloudflare when the website is used.
Purpose and consent
The details are used to record, review and respond to the request. The form requires an unticked consent box before submission and explains the current service status at the point of collection.
Storage and deletion
Enquiries are stored in a Cloudflare D1 database in the website's Cloudflare account. Records are automatically removed after 30 days through cleanup performed by the enquiry service. The site does not use the form data to build advertising audiences.
Sharing
No provider is currently connected and no automatic sharing occurs. This notice must be updated before any referral partner receives data. The person submitting would need to be told who will receive it and for what purpose.
Your choices
You may ask the website operator to locate, correct or delete an enquiry by using the contact page and quoting the short reference shown after submission. Formal operator identity and contact details will be added as part of the owner's separate compliance process.
